NYSBA Dental Program from United Concordia

NYSBA members have access to a comprehensive dental program exclusively designed for New York legal professionals.  With competitive rates and three comprehensive plan options to choose from, the NYSBA dental program offers options to suit your dental needs and your budget.  Plans are available to both sole proprietors and groups of 2 or more.

Option 1:
 Download Option 1 Benefit Summary
• 100% coverage for Basic  (Class I) services - i.e. cleanings, X rays, exams
• 80% coverage for Class 2 services – i.e. spacers, simple extractions
• 50% coverage for Class 3 services - i.e. crowns, bridges, complex oral surgery
• $1,000 calendar year maximum per individual
• $50 Deductible per person/$150 max deductible per family
• Available to add to the medical program or as a stand alone benefit

Option 2:
 Download Option 2 Benefit Summary 
• 100% coverage for Basic (Class I) services - i.e. cleanings, X rays, exams
• 90% coverage for Class 2 services – i.e. spacers, simple extractions and any procedures requiring anesthesia.  Class II services have also been expanded to include Complex oral surgery and surgical periodontics.
• 60% coverage for Class 3 services - i.e. crowns, bridges, dentures
• 50% coverage for Orthodontics to $1,500 lifetime maximum
• $1500 calendar year maximum per individual
• $50 Deductible per person/$150 max deductible per family
• Available to add to the medical program or as a stand alone benefit

Option M:

• 100% coverage for basic (Class I) services - i.e. cleanings, X rays, exams
• 70% coverage for Class 2 services – i.e. spacers, simple extractions
• No coverage for Class 3 services - i.e. crowns, bridges, complex oral surgery
• $800 calendar year maximum per individual
• No Deductible
• Only available to members who have enrolled in the MVP medical program
